Ohev Sholom - The National Synagogue

I understand that Ohev Sholom – The National Synagogue and their staff/volunteers are providing a roller skating rink and skates in the synagogue Social Hall.

 I HEREBY WAIVE AND RELEASE, Ohev Sholom – The National Synagogue (“OSTNS”), and their staff/volunteers, from any and all lawsuits, damages and liabilities, under any theory or cause of action(including but limited to claims based on strict liability, negligence, gross negligence or intentional acts)  that I have or may have, arising  out of or related to  our participation  in rollerskating and/or hockey at the synagogue.

 I understand that the activities my child will participate in with roller- skating have the potential to be dangerous. By this Waiver, I assume any risk and waive any claims of personal injury arising from the participation of my child in rollerskating and/or hockey at the synagogue.

 I agree to indemnify and to hold OSTNS and their staff/volunteers free from any liability including attorneys' fees and costs of defense, which may be incurred as the result of my child’s participation in rollerskating and/or hockey at the shul.

 I have read, understand and agree to the terms of this WAIVER AND RELEASE. I have signed this agreement freely and voluntarily.

 District of Columbia law applies to this agreement. The parties agree that the District of Columbia is a correct venue and jurisdiction in which to enforce this agreement.

I understand that Ohev Sholom – The National Synagogue and their staff are providing an event that utilizes large inflatable games for children.

I HEREBY WAIVE AND RELEASE, Ohev Sholom – The National Synagogue (“OSTNS”), and their staff, from any and all lawsuits, damages and liabilities that I have or may have, arising from our use of the inflatable games or activities my child may be involved.

I understand that the activities that my child will participate in with the inflatables have the potential to be dangerous. By this Waiver, I assume any risk and waive any claims of personal injury arising from the use by me or my child of the inflatables owned or used by OSTNS.

I agree to indemnify and to hold OSTNS and their staff and volunteers free from any liability, which may be incurred as the result of my, or my child(ren)’s use of the inflatable games.

I have read, understand and agree to the terms of this WAIVER AND RELEASE. I have signed this agreement freely and voluntarily. I understand that I need to assume responsibility watching and caring for my child or minor’s safety.

DC law applies to this agreement. The parties agree that the District of Columbia is a correct venue and jurisdiction in which to enforce this agreement.

I give OSTNS’ Camp Kibbutz permission to transport my child on all field trips by bus.

I give permission and consent to allow photographs to be taken of my child(ren) during camp session activities. I further give permission and consent that any such photographs may be published and used by Ohev Sholom - The National Synagogue & Camp Kibbutz to illustrate and promote the camp experience, Ohev Sholom and its camp programs.
